OT Assessments and Reports

Wonderfully OT aims to provide clear and thorough Functional Capacity Assessments for children and young people up to 18-years-old. Every child - and every family - deserves to have their goals, strengths and challenges well understood. When these are clearly seen, they can be best supported in taking their next steps.

 

Functional Capacity Assessments are often required for NDIS purposes. This can include for plan reviews, for significant changes in circumstances or if otherwise requested by yourself or your team.

Home visiting is available in the Northern Brisbane Suburbs/The Moreton Bay region. 

Telehealth assessments are also available depending on your needs.​​​​​​

Wonderfully OT can cater to a range of report writing and assessment needs for young people aged between 4 to 18-years-old. This includes:

  • NDIS Access reports

  • Summaries and recommendations for schools, childcare and other facilities. 

  • Summaries for medical professionals such as Paediatricians and GPs. 

  • Parent-friendly summary reports that capture your child's strengths, development and needs.

 

Wonderfully OT aims to cater to the individual requests of families while delivering assessments and reports to help children to overcome their challenges and thrive in their environments. These assessments aim to be concise and easily understood so as to fulfil their specific purpose. 

Please note there are assessment services that are outside the scope of Wonderfully OT. This includes equipment prescription and family court reports.

Although Wonderfully OT already offers a motor assessment as part of our comprehensive assessments, we are looking to 'level up' this process.

Why?

  • To provide an accessible service for families who specifically want to have their child screened for Developmental Coordination Disorder and other similar challenges*. 

  • To offer clear and evidence-informed guidance based on assessment findings that can be easily integrated into everyday life.

 

Who is this for?

  • Children who are struggling with their confidence in movement, they may tire easily, appear 'clumsy' or uncoordinated.

  • Families seeking clarity around motor coordination concerns

 

What will this new assessment include?

  • Completion of the Movement-Assessment Battery for Children - this is a 'gold-standard tool' used to identify motor coordination difficulties and screen for Developmental Coordination Disorder (DCD)

  • A clear, easy to understand explanation of your child's movement strengths and challenges

  • Practical recommendations for home and school

  • Guidance on next steps 

When will this service be available?

  • September 2026 onwards

*Please note that Developmental Coordination Disorder and other similar challenges are formally diagnosed by your Paediatrician. As part of this process, an OT assessment alongside other health professionals may be requested.

​If you're interested in booking a service with Wonderfully OT, follow the steps below:

  1. Fill out our enquiry form at the end of the page or email maddie@wonderfullyot.com.au 

  2. You will receive an information pack based on your enquiry, an intake form and our privacy policy​.

  3. When you have returned your intake form, you will be sent a service agreement which includes a personalised quote based on your needs. 

  4. Once everything is in place, you will be offered an appointment time and date.
