OT Assessments and Reports
Wonderfully OT aims to provide clear and thorough Functional Capacity Assessments for children and
young people up to 18-years-old.
Every child - and every family - deserves to have their goals, strengths and challenges well understood.
When these are clearly seen, they can be best supported in taking their next steps.
Functional Capacity Assessments are often required for NDIS purposes.
This can include for plan reviews, for significant changes in circumstances or if otherwise requested by yourself or your team.
Home visiting is available in the Northern Brisbane Suburbs/The Moreton Bay region.
Telehealth assessments are also available depending on your needs.
Wonderfully OT can cater to a range of report writing and assessment needs for young people aged between 4 to 18-years-old. This includes:

NDIS Access reports

Summaries and recommendations for schools, childcare and other facilities.

Summaries for medical professionals such as Paediatricians and GPs.

Parent-friendly summary reports that capture your child's strengths, development and needs.
Wonderfully OT aims to cater to the individual requests of families while delivering assessments and reports to help children to overcome their challenges and thrive in their environments. These assessments aim to be concise and easily understood so as to fulfil their specific purpose.
Please note there are assessment services that are outside the scope of Wonderfully OT. This includes equipment prescription and family court reports.

Does your child find movement activities difficult?
Sometimes children need more support, time and opportunities for skill development to move and position their bodies for everyday activities. In order to provide this support, understanding what is going on under the surface is an important first step.
Our Occupational Therapy Movement Assessment is for families who may notice that their child/ren:

Finds learning new physical skills difficult,
-
Avoids sports, playground activities or other movement-based activities,
-
Becomes frustrated, needs extra time or tires easily when completing activities,

Has difficulty with handwriting, drawing, using scissors, completing craft activities, doing buttons, tying shoelaces, eating with cutlery etc.

Appears to have difficulty organising themselves and starting activities,
-
Has reduced confidence when they aren't able to keep up or complete the same activities as their peers...
What is the Occupational Therapy Movement Assessment?
An Occupational Therapy movement assessment is designed for children from 6-years of age up to young adults (21-years-old). The assessment aims to provide clarity in understanding movement challenges, and help to identify what strengths can be harnessed and what areas of difficulty they may need additional support to develop.
In this assessment we consider what underlying skills can contribute to movement challenges such as:

Fine motor challenges

Motor planning/praxis
-
Sensory processing and sensory-motor integration
-
visual perception and visual-motor integration
-
Executive functioning challenges
Using standardised motor assessments (such as the BOT-II and Movement-ABC 3) alongside other assessments, this service aims to provide more clarity on how the above skills impact your child's ability to complete everyday tasks.
This service intends to offer support alongside your child's team of professionals. Referral to other professionals such as Physiotherapy may be recommended if required.
What does this assessment include:

Initial intake over the phone (up to 30-minutes),

One to two assessment sessions in your home (Moreton Bay region),

A short summary report of key findings from the assessment and a recommended plan to address identified challenges,

Phone consultation to discuss results and next steps.

You will receive an information pack based on your enquiry, an intake form and our privacy policy.

When you have returned your intake form, you will be sent a service agreement which includes a personalised quote based on your needs.

Once everything is in place, you will be offered an appointment time and date.
